Community Connector (CC)

BethelCare’s Community Connector (CC) services provide individualized, community based support designed to help individuals build independence and actively participate in their community. Services are personalized to meet each person’s goals and may include assistance with accessing local resources, developing life skills, strengthening social connections, and increasing community involvement. Supports are flexible and focused on promoting confidence, inclusion, and meaningful participation in everyday life.
